- Obtain a new vaccine card or copy of vaccine record: There are several options for you to replace your CDC COVID-19 vaccination card.
- Access COVID-19 treatment: Learn about how and where to access COVID-19 treatment.
If you don't have health insurance or a regular doctor, call the County's Patient Access Department at 1(866) 967-4677 for assistance.

Schools: For inquiries related to schools, including childcare, daycare, kindergarten, and K-12, visit the COVID-19 and Schools/Childcare page or call the Santa Clara County Office of Education Hotline at (408) 453-6819.
The County of Santa Clara Isolation and Quarantine Support Program provides a motel room and three meals per day for COVID-19 cases who do not have a home and do not reside in a congregate shelter.
